Looking to Enroll your Student(s) at Avon CSD?

Please call or email the District Registrar with the following:

  • Your Name

  • Address

  • Phone Number

  • Email Address

  • Your Student(s) Name(s)

  • Birthdate(s)

  • Grade(s) Entering

  • Current School District

Once you have sent the above information to the District Registrar, please complete the New Student Registration Packet for each student you wish to enroll, and include the following documentation:

  • Student Birth Certificate or Passport

  • Guardianship Verification (if necessary)

    • Separation Papers

    • Custody Agreement

      • Please Note: Only court odered documents will be accepted

  • Proof of Residency

    • Photo Driver's License ID of Parent/Guardian with Updated Address

    • Mortgage Agreement

    • Lease Agreement

    • Payroll Stub with Updated Address

    • Utility or Cable Bill with Updated Address

    • Auto Insurance Policy with Updated Address

  • Up-To-Date Health Information

    • Immunization Record - It is a New York State requirement that parents/guardians MUST provide the school with a copy of their child's immunization record from the child's Physician's Office.  Further, if no immunization record is provided, the student will not be able to start school until the parent/guardian can provide the information.  For more information regarding NYS Immunizations please review the following: Required Immunizations, NYSDoH Information for Parents

    • Copy of Most Recent Physical - NYS school physicals are valid for 1 full year.  If your child's most current physical has expired, a new physical will need to provided to the District within 14 days of your child's first day of school.  For out of state students entering the District, a physical administered by a NYS physician is required and to be provided to the District within 30 days of your child's first day of school, regardless if they have a valid current physical from a previous physician in a previous state.

Please return the completed packet(s) directly to the District Registrar located in the Business Office.
If the registrar is unavailable, please leave the packet(s) with the Middle School Secretary. You may also call and schedule an appointment to drop off your packet(s) and meet with the registrar.
