You will have the option to upload documents during the online registration process. If you prefer not to upload documents, the registrar will contact you once the registration has been received to make an appointment to hand in the documentation.

Guardianship Verification (if applicable)
Proof of Residency
Copy of a residential lease or proof of ownership of a house or condominium, such as a deed or mortgage statement.
Statement by a third-party landlord, owner or tenant from whom the parent(s) of person(s) in parental relation leases or with whom they share property within the District
Such other statement by a third-party establishing the parent(s) or person(s) in parental relation's physical residence presence in the District.
If none of the about are available, the District will consider other forms of documentation and information which may include, but will not be limited to, the following:
Pay stub, recent utility bill, or income tax form
Driver’s license, learner’s permit or non-driver identification
Documents issued by federal, state or local agencies; state or local government issued identification
Membership or voter registration documents based on residency
Judicial custody or guardianship papers
Up-to-Date Health Information
Immunization Record
Physical Examination
