Looking to Enroll your Student(s) at Avon CSD?

Please call or email the District Registrar with the following:

  • Your Name

  • Current Address

  • New Avon Address (if different from current address)

  • Phone Number

  • Email Address

  • Student(s) Name(s)

  • Birthdate(s)

  • Current Grade(s) or Grade(s) Entering

  • Current School District

Once you have sent the above information to the District Registrar, a New Student Registration Packet will be mailed to you for each student you wish to enroll. If you prefer the packet be emailed, please be sure to make that request in your original email.

The following documents need to be provided at the time of registration:

  • Student Birth Certificate or Passport

  • Guardianship Verification (if necessary)

    • Separation/Divorce Papers

    • Custody Agreement

  • Proof of Residency

    • Photo Driver's License ID of Parent/Guardian with Updated Address

    • Mortgage/Lease Agreement

    • Payroll Stub with Updated Address

    • Recent Utility or Cable Bill with Updated Address

    • Current Auto Insurance Policy with Updated Address

  • Up-To-Date Health Information

    • Immunization Record - It is a New York State requirement that parents/guardians MUST provide the school with a copy of their child's immunization record from the child's Physician's Office.  Further, if no immunization record is provided, the student will not be able to start school until the parent/guardian can provide the information.  For more information regarding NYS Immunizations please review the following: Required Immunizations, NYSDoH Information for Parents

    • Most Recent Physical - NYS school physicals are valid for 1 full year.  If your child's most current physical has expired, a new physical will need to provided to the District within 14 days of your child's first day of school.  For out of state students entering the District, a physical administered by a NYS physician is required and to be provided to the District within 30 days of your child's first day of school, regardless if they have a valid current physical from a previous physician in a previous state.   • Transportation Forms - Transportation forms must be completed for ALL students. This includes walkers and parent pick-up/drop-off.

  • Technology Acceptable Use Forms

Once the packet is completed, please call or email me to schedule an appointment to drop off your paperwork.

Walk-ins are welcome, but I may not be available at the time you come in and will ask that you contact me to make an appointment.
