New Student Registration Process:

Please be sure to include the following items when registering your child(ren): 

   *Original Birth Certificate or Passport (a copy of the document will be accepted at this time)
  
 *Guardianship Verification (if necessary)
           -Separation Papers
           -Custody Agreement
    *Proof of Residency, acceptable forms of proof of residency are:
         
-Photo Driver's License of Parent/Guardian with current address
          -Mortgage Agreement
          -Current Lease Agreement
          -Recent Payroll stub of Parent/Guardian
          -Recent Utility or Cable Bill
          -Current Auto Insurance Policy
    *Up-to-Date Health Information
         -Immunization Record - it is a New York State requirement that parents/guardians MUST provide the school with a copy of their child's immunization record from the child's Physician's office.  Further, if no immunization record is provided, the student will not be able to start school until the parent/guardian can provide the information.  For more information regarding NYS Immunizations please use these links: Elementary School Health Office5-12 Health OfficeHealth Forms/Information for New Students
         
-Copy of most recent physical - NYS school physicals are valid for 1 full year.  If your child's most current physical has expired, a new physical will need to provided to the District within 14 days of your child's first day of school.  For out of state students entering the District, a physical administered by a NYS physician is required and to be provided to the District within 30 days of your child's first day of school, regardless if they have a valid current physical from a previous physician in a previous state.  For more information regarding school physical requirements please refer to this link: 2020-2021 Health Requirements.  If your child is interested in participating in sports, please visit the following site for health requirements: 7-12 Sports Participation
