MEDICAL INFORMATION:

  • Physicals are required by New York State for new entrants, students in Kindergarten, Grades 1, 3, 5, 7, 9, and 11. Please schedule your child physicals as soon as possible. The cut off date is September 2013, 12 months prior to the beginning of the upcoming school year. When complete please bring, mail, or fax the form to my office. My fax number is 226-1709.

  • Children should have a physical done every year around their birthday. ALWAYS keep a copy of your child's physicals in a safe place at home.

  • Do not assume your doctors office will fax the school a copy. Please drop off a copy in person or put a copy in a sealed envelope and put in your child's take home folder. The teachers will make sure that I get them. Too often I do not receive the fax from the doctor's office.

  • Immunizations should be as complete as possible when school starts. In accordance with Public Health Law #2164, all children entering school must be immunized against polio, measles, rubella, mumps, hepatitis B and diptheria. Proof must be presented at the time of registration that these immunizations are completed or in process of being completed. If booster shots haven’t been given, please let the school nurse know the scheduled date.

  • A law was recently passed that expands health screenings to include the dental health of all students in New York State. Proof of a dental exam is requested for new entrants, Kindergarten, 1, 3, 5, 7, 9, & 11. The exam must be completed no more than one year prior to the current school year. A certificate needs to be handed in once the examine is completed.

  • BMI Recordings -

    As part of a required school health examination, a student is weighed and height is measured. These numbers are used to calculate the student's Body Mass Index (BMI).

    NYS Education Law require that BMI and weight status group be included as part of the student's school health exam.

    The NYS Department of Health sometimes require us to report student weight status groups. Only summary information is sent. NO NAMES OR INFORMATION ABOUT INDIVIDUAL STUDENTS ARE SENT. However, you may choose to have your child's information excluded from this survey report.

    If you do not wish to have your child's weight status group information included as part of the Health Department survey this year, please send your request in writing to the school nurse.
