Health Information for New Students

For the 2021-2022 school year, New York State law requires a health examination for all students entering the school district for the first time and when entering grades Pre-K or K, 1st, 3rd, 5th, 7th, 9th, and 11th grade. The examination must be completed by a New York State licensed physician, physician assistant, or nurse practitioner.

The district will not permit a student lacking proof of physical examination or immunizations to remain in school for more than 14 days, or more than 30 days for an out-of-state or out-of-country transferee. If a student has an appointment beyond the 14 day or 30 day period, proof of appointment must be provided to the Health Office.

A dental certificate which states your child has been seen by a dentist or dental hygienist is also asked for at the same time.

We suggest you make copies of the completed form for your own records before submitting them with your registration packet. If you have any questions, please either contact the registrar, Crystal Ryan or contact the health office.
