2020-2021 Kindergarten Registration

Dear Parent or Guardian:

 Below you will find the kindergarten materials that will need to be completed for your incoming Kindergarten student.   Thank you very much for completing these documents and returning them during our kindergarten registration   period. All forms must be filled out entirely. If you have any questions, you may contact us at school at 226-2455,   extension 1110.

The following documents must be completed or provided to the school before a student can begin attending:

  • Birth Certificate or other form of proof of birth

  • Registration/Emergency Health Form

  • Proof of address showing Avon residency (ie. recent utility bill, driver's license, lease or mortgage agreement, recent pay stub)

  • Guardianship verification (if necessary, ie. divorce/custody papers or other legal documentation)

  • Most recent NYS Health Exam/Physical and Immunizations

  • Kindergarten Health Questionnaire 

  • Dental Health Certificate

  • Student Racial and Ethnicity Identification

  • Housing Questionnaire 

  • Home Language Questionnaire

  • Migrant Parent Survey

Below is a list of the key events and dates for you and/or your incoming kindergartner to attend: 

Kindergarten registration will take place the week of March 2nd (Monday through Friday) between the hours of 10:00 AM and 4:00 PM in the elementary school main office.  You may also register Wednesday, March 4 from 4:00 to 6:00 PM.    
Parent Information Night will be Monday, May 4th in the Elementary School library at 6:00 PM.  This is for parents and guardians only.  Our kindergarten teachers will be there to introduce themselves and to discuss your child’s first year at Avon.  There will also be a time for questions and answers.  

Kindergarten screening for incoming kindergarten students will take place at the end of June.  Screening takes approximately one hour.  You will sign up for screening at the parent information night.  During screening students are given a number of tasks to complete. This information gathering helps us to learn about your child’s learning strengths and areas in need of development.  This is our way of getting to know your child as a learner. 

Kindergarten Summer Orientation takes place at the end of the summer, just prior to the opening of school.   This gives children a chance to walk through the halls with their parents, go to their classrooms and meet their teachers.  Parents are notified of the teacher assignment and the date of the Summer Orientation in early August.
